Purpose: Process of uploading AOPs, Waivers of Paternity, and Divorce Decrees to the Document Library

Sequence

Step Description

1. Review
  • Review the electronic folder in HPP Team Goals
  • Start with the oldest file
  • Ensure the file is opened in Adobe
2. Data Entry
  • Review AOP to ensure accuracy
  • Complete data entry in AZCARES (See Data Entry Standard Work)
  • Once data entry is completed, retrieve the HPP Person ID number and leave the HPP Acknowledgment tab open.
3. Prepare AOP
  • Return to the Adobe electronic file
  • Print the document (Click printer symbol or Ctrl P)
  • Destination - select Microsoft Print to PDF
  • Pages - custom
  • Enter page numbers
  • Click Print
  • File name - AOP (Child’s Name)
  • Save
  • Is there a Waiver of Paternity Affidavit or Divorce Decree attached?
  • If yes, complete the following steps:
  • Print the document (Click printer symbol or Ctrl P)
  • Destination - select Microsoft Print to PDF
  • Pages - custom
  • Enter page numbers
  • Click Print
  • File name - Waiver or Divorce Decree (Child’s Name)
  • Save
4. Upload
  • Return to HPP Acknowledgments in AZCARES
  • Select Upload Document (Verify that information is populated; look for orange tv if not populated, exit and click Upload Document again.)
  • Document Type - CS-127 Acknowledgment of Paternity
  • File Name - AOP (Child’s Name)
  • Next
  • Upload Files or Drop File (Select AOP saved in step 3)
  • Finish
  • If there is a waiver or divorce decree, complete the following steps:
  • Select Upload Document (Verify that information is populated; look for orange tv if not populated, exit and click Upload Document again.)
  • Document Type - Waiver of Paternity Affidavit or DCSS Correspondence
  • File Name - Waiver of Paternity Affidavit or Divorce Decree
  • Next
  • Upload Files or Drop File
  • Select Waiver or Divorce Decree saved in step 3.
  • Finish
5. Quality Check
  • Go to Document Library
  • Enter HPP Person ID into the HPP Case ID field from step 2
  • HPP Case ID will populate
  • Search
  • View the file to ensure the correct AOP, Waiver, or Divorce Decree is uploaded
  • Complete
